    The Final Stretch: A Doctor’s Guide to the Third Trimester

    Welcome to the third trimester (weeks 28-40)! You are in the home stretch, and the anticipation of meeting your baby is growing every day. During these final weeks, your baby is continuing to grow and mature, and your body is making its final preparations for labour and delivery. As a GP, I’m here to guide you through what to expect in this final, exciting chapter of your pregnancy.

    What to Expect in the Third Trimester

    As your baby grows, you will likely experience a new set of physical symptoms. While some can be uncomfortable, they are a normal part of this stage of pregnancy.

    Common Third Trimester Symptoms:

    • Backaches: Your growing belly shifts your centre of gravity, which can put a strain on your back. A supportive maternity belt can provide relief.
    • Shortness of Breath: Your expanding uterus puts pressure on your diaphragm, which can make it feel harder to take a deep breath.
    • Heartburn: Hormonal changes relax the valve between your stomach and oesophagus, which can allow stomach acid to leak back up.
    • Braxton Hicks Contractions: These are “practice” contractions that feel like a tightening or hardening of your abdomen. They are typically irregular and not painful.
    • Swelling: You may notice swelling in your hands, feet, and ankles. Elevating your feet and staying hydrated can help.

    Key Preparations for Birth

    Now is the time to finalise your preparations for your baby’s arrival.

    Preparation Action
    Pack Your Hospital Bag Have your bag packed and ready to go by week 36.
    Install the Car Seat Make sure your baby’s car seat is properly installed in your car.
    Attend a Birth Class A good birth class can help you and your partner feel more prepared for labour and delivery.
    Discuss Your Birth Preferences Talk to your doctor or midwife about your preferences for pain relief, labour positions, and other aspects of your birth experience.
    Prepare for Postpartum Stock your home with postpartum recovery essentials like maternity pads and comfortable loungewear.

    Recognising the Signs of Labour

    One of the biggest questions in the third trimester is, “How will I know I’m in labour?” While every woman’s experience is different, there are some common signs that labour is starting.

    Early Signs of Labour:

    • The “Show”: You may lose your mucus plug, which can look like a small amount of blood-tinged mucus.
    • Lightening: You may feel the baby “drop” lower into your pelvis. This can relieve shortness of breath but may increase pressure on your bladder.
    • Contractions that are Regular, Stronger, and Closer Together: This is the most reliable sign of true labour. Time your contractions. If they are coming at regular intervals and getting progressively stronger and closer together, it’s time to call your doctor or midwife.
    • Your Water Breaking: This can be a large gush or a slow trickle of fluid. If you think your water has broken, contact your healthcare provider.

    Enjoy these final weeks of pregnancy. Rest as much as you can, nurture your body with good food and gentle movement, and take time to connect with your baby. The journey of pregnancy is nearing its beautiful conclusion, and the incredible adventure of motherhood is about to begin.
